Failure Modes and Effects Analysis (FMEA) and Failure Modes, Effects and Criticality Analysis (FMECA) are methodologies designed to identify potential failure modes for a product or process, to assess the risk associated with those failure modes, to rank the issues in terms of importance and to identify and carry out corrective actions to address the most serious concerns.
In RCM++, you can use the FMEA tab of the Analysis panel to record the functional failure analysis for each piece of equipment that will be analyzed with Reliability Centered Maintenance techniques. For this purpose, the analysis typically includes a list of the functions the item is intended to perform, the ways in which the item may fail to perform its intended function, the consequences of failure and the specific failure modes that may need to be addressed with an effective maintenance strategy.
 
You can also use this tab for traditional FMEA or FMECA, if desired. For this purpose, the analysis typically includes a list of the functions the item is intended to perform, the potential failure modes, the effects of failure and the specific causes that may need to be addressed with corrective actions or prevention/detection controls in order to reduce the risk in the design.

Functional Failure Analysis
